#3. Please be advised there are no responsive documents for items #1 and
#2. As stated in the agreement “the contractor shall submit to the public
agency, after notification of award but prior to execution of a goods and
services contract, one of the following three (3) documents:
Letter of Federal Affirmative Action Plan Approval
Certificate of Employee lnformation Report
Employee lnformation Report Form A4302”
The submitted document from the vendor is the “Employee Information
Report”.
