Under the New Jersey Open Public Records Act, N.J.S.A. 47:1A-1 et seq., I would like the opportunity to obtain or inspect public records concerning calls made from the specified addresses below to request EMS or paramedics services and the resulting reports. I am interested in records beginning January 1, 2019 to December 31, 2021 specific to each address, but without revealing any personally-identifiable health information. Documents could include call logs, incident reports, and other documents related to the 911 calls and medical services response that include the date and time of the incident, location from where the call was made, the responding entity, the nature of the call and, if possible, a brief description of the incident.
If this is not the appropriate place to send such a request, please identify the organization which serves as the Public Service Answering Point for the 911 emergency medical response calls in your area. If any other agency or organization also maintains any such records related to each of these emergency medical services, please identify the agency or organization, and the responsible person to whom I could address a similar request.
