Under the New Jersey Open Public Records Act, N.J.S.A. 47:1A-1 et seq., I would like the opportunity to obtain or inspect public records concerning calls made from the specified addresses below to request EMS or paramedics services and the resulting reports. I am interested in records beginning January 1, 2020 to the present specific to each address, but without revealing any personally-identifiable health information. Documents would likely include call logs or incident reports, as I received these from other New Jersey townships.
If this is not the appropriate place to send such a request, please identify the organization which serves as the Public Service Answering Point for the 911 emergency medical response calls in your area. If any other agency or organization also maintains any such records related to each of these emergency medical services, please identify the agency or organization, and the responsible person to whom I could address a similar request.

If there are any fees for searching or copying these records, please inform me if the cost will exceed $100.
